Hopscotch Primary Care Raises $53M to Expand Its Primary Care Services

Hopscotch Primary Care, a technology-enabled primary care provider for patients in rural communities, has raised $53 million in its Series D funding round to expand its healthcare services across new communities in the United States.

8VC and Townhall Ventures co-led the investment round with participation from existing investors, including aMoon Fund, Citi Impact Fund, Alumni Ventures, K2 HealthVentures, the Autism Impact Fund (AIF), John Doerr, Chairman of Kleiner Perkins, Dr. Richard Merkin, Founder of Heritage Provider Network, and the Leon Levine Foundation.

Founded in 2021, the company serves more than 15,000 patients across the Southeastern United States, with its largest concentration in rural Western North Carolina. The company’s mission is to transform lives in rural communities through accessible and proactive value-based care.

Hopscotch Primary Care secured its latest Series D funding 2 years after closing its $50 million Series C round. The latest funding from top-tier institutional backers underscores that Hopscotch Primary Care has built a highly scalable, repeatable, and commercially viable model capable of solving the chronic healthcare access crisis in rural America. And with fresh funds, the company plans to deepen its market footprint and scale its data-driven and AI-powered healthcare operations.

“This new funding comes at an inflection point for Hopscotch, and for rural health care as a whole. We are accelerating our growth into more small towns while improving outcomes, delighting patients, and building a sustainable future for the communities we serve,” added Mr. Gronniger.

Gartner research highlights that artificial intelligence and unified data platforms are accelerating the shift toward value-based primary care.

Healthcare organizations are adopting autonomous AI and cloud data stacks to improve risk stratification, manage contracts, and streamline care coordination, according to the latest Gartner report.  

According to our latest funding report, digital health startups, including value-based care providers, have raised $20 billion in funding so far this year.

More recently, Aligned Marketplace has secured an additional $20 million in a Series A round to meet the growing demand for its advanced value-based primary care model in the growing US market.

Karoo Health, a value-based heart care company, raised $16.2 million in Series A funding, co-led by 7wire Ventures and Allumia Ventures, with participation from First Trust Capital Partners, LLC, SpringRock Ventures, Hyde Park Angels (HPA), and other marquee investors.
